Written Reports and Travel$140/hour *Fee is prorated for time used Telephone Contact$140/hour (free, if less than 15 minutes) *Fee is prorated for time used FeesFees are paid at the beginning of each visit. Payments can be made in cash or check. If a check is returned due to insufficient funds, a reprocessing fee of $10 will be charged and paid at the beginning of the session. If you want a receipt for your payment, please ask for one in the beginning of the session. A fee increase typically occurs at most 2x/year which only affects you if you are not using your health insurance. If you have a concern about the ability to pay the fee, please feel free to bring it up to me. Usually, a mutually agreeable arrangement can be worked out and reviewed every 3 or 6 months. Missed Appointments and Late CancellationsWhen you schedule an appointment with me, you are “purchasing” that time. Clients who use health insurance and do not cancel appointments within 24 hours are charged half of the full contracted fee that I have with their particular health insurance for the first late cancelled/missed session, and full contracted fee for any subsequent missed sessions, to be paid at the next session. Please remember that insurance companies will not reimburse for missed sessions or late cancellations. You will be responsible for this payment. Privately paying clients are charged $60 for the first late cancelled/missed session and full fee for any subsequent missed sessions, to be paid at the next session. This policy applies to an appointment you did not cancel because you have decided not to continue counseling, an appointment you “forget”, or an appointment which conflicts with another one you have made, of if you choose to do something that is important to you rather than come to counseling. It is your responsibility to contact me if you are unsure when your appointment is scheduled. I do not call to give appointment reminders. The only exception to the 24 hour cancellation policy is if there is a true emergency. “Emergencies” are considered events beyond your control such as snowstorms, car accidents, funerals, hospitalizations, or illnesses of the degree which keep you out of work. TardinessPlease call if you are running late for an appointment. Regardless of how late you show up for the appointment, the appointment will end on-time. However, on the occasion that I am running late, we will make up the time by extending the end of the session, or begin early or end late at the next session. Termination of ServicesIf at some point, you decide not to continue in counseling with me, please call me and leave a message, especially if you have appointments scheduled. I will have considered you ‘dropped out’ of treatment if you do not respond to my phone calls. If you have not had contact with me for 3 weeks, a final letter will be sent stating that your case has been closed. If I determine that you need a different type of psychotherapy care, I will discuss your needs with you and make a transfer to another provider. Lastly, if together you and I decide that your treatment goals have been met, we will discuss if closing your case is warranted. InsuranceIf you are hoping your insurance will reimburse a service, please check with your insurance company before we meet. Please notify me of any changes to your insurance or contact information. You are responsible for any charges that occur as a result of such a change. On January 1, 2001, the Massachusetts Health Parity Law went into effect for Massachusetts residents only. “Parity” refers to mental health being guaranteed coverage on a “par” or to the same degree as physical illness. What this may mean to you: If the Parity Law applies to your insurance and to your mental health condition, the number of sessions you are allowed in a calendar year is unlimited. If you have any questions, please call your insurance company to determine if and when the Parity Law will apply to your policy. |
